Claim forms are now available in both online and paper versions as of January 4, 2022. 

The online form is found at and can be completed entirely online.   

If you would prefer a paper version, please contact the Claims Administrator (Deloitte) to request that a paper claim form be mailed to you. Class Members who complete paper forms will have to mail or fax them directly to the Claims Administrator. The Claims Administrator may be reached at or 1-877-877-5786.  

Class Members should only submit one Claim Form. Submitting multiple claim forms may lead to processing delays.
